The Bridging the Gap Project is designed to enhance transitions from the hospital to home for children birth to 3, who are eligible or potentially eligible for North Carolina Early Intervention services.

Bridging the Gap staff and volunteers provide early intervention information to families of children in Neonatal Intensive Care and Pediatric Intensive Care Units in several North Carolina hospitals.
